The VA Aid & Attendance benefit is one of the most underused senior benefits in the country. For qualifying Iowa veterans and their surviving spouses, it can pay $1,400 to $2,500+ per month toward assisted living, memory care, or in-home care — on top of regular VA pension. Most Iowa seniors who qualify don't even know the benefit exists. Here's what you need to know.

Aid & Attendance (A&A) is an enhanced VA pension benefit for wartime veterans and their surviving spouses who:
1. Served during a recognized wartime period (at least 1 day during wartime; 90 days total active duty) 2. Were discharged under conditions other than dishonorable 3. Meet income and asset limits 4. Need help with activities of daily living (ADLs) — bathing, dressing, eating, toileting, medication management — OR are bedridden, legally blind, or in a nursing home
It is NOT a disability benefit (different program). It's a pension supplement.

These amounts are reduced dollar-for-dollar by countable income above the limit. But crucially, unreimbursed medical expenses (including assisted living and memory care costs) reduce countable income, which means most seniors in care facilities qualify for the full benefit.
The veteran doesn't need to have served in combat — just during a wartime period. Current recognized periods:
So virtually any veteran who served in the US military during or after WWII qualifies on the service-period requirement.
Asset limit (2026): Approximately $155,000 in countable assets, excluding:
Income limit: The formula is complex — effectively, countable income minus unreimbursed medical expenses must be below the Maximum Annual Pension Rate (MAPR). Because assisted living and memory care costs are unreimbursed medical expenses, most applicants in care facilities have effectively negative "countable income" and qualify for the full benefit.
In 2018 the VA added a 36-month look-back period for asset transfers. If you gave away assets in the 3 years before applying, the VA assesses a penalty period during which benefits are reduced or denied.
This is shorter than the Medicaid 5-year look-back but longer than most families realize. Do not transfer assets hoping to qualify for A&A unless you've planned for it 3+ years in advance.
1. Confirm eligibility — talk to a VA-accredited attorney, claims agent, or veterans service officer (free help) 2. Gather documents: - DD-214 (military discharge) - Marriage certificate (for spouses and surviving spouses) - Death certificate (for surviving spouse claims) - Medical records showing need for assistance with ADLs - Financial records (bank statements, brokerage accounts, SSA-1099, property valuations) 3. File VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance) — completed by the veteran's doctor 4. File VA Form 21P-527EZ (pension application) OR VA Form 21P-534EZ (surviving spouse pension application) 5. Submit to the VA Pension Management Center for your region 6. Wait for processing — currently 4-8 months
Benefits are retroactive to the application filing date, so file as early as possible even if you're not sure you qualify.
